OnTrack Health™ is a customizable web and app-based platform that collects Patient Reported Outcomes (PRO) as well as information from medical devices. The platform offers AI-driven solutions for Device Manufacturers to improve their device offerings. OnTrack Health™ trains medical professionals using real outcome data and offers training courses to physicians as needed. The platform also allows Device Manufacturers to engage physicians and assess their knowledge of a device using game theory. Platform’s one of a kind innovative QR Code Solution brings Device Manufacturers, Surgeons and Patients together so that Device Manufacturers and Hospitals/Clinics/Private Practices can get a comprehensive view as patients move through their treatment pathway.
OnTrack Health™ offers an image-guided AI-driven surgeon training module and a unique QR Code solution that brings Device Manufacturers, Surgeons, and Patients together. This allows for a comprehensive view of patients as they move through their treatment pathway, enabling more efficient and effective collaboration between Device Manufacturers, Hospitals, Clinics, and Private Practices.
OnTrack Health™ uses a unique AI solution to predict and proactively address potential surgery no-shows, resulting in an impressive 18% reduction in no-show rates, leading to substantial cost savings for hospitals and clinics.
The OnTrack Health™ (OTH) platform started with oncologic surgeons who wanted to improve the surgical care of their prostate cancer patients. The proposed idea: a timed pre- to post-surgical pathway incorporating education material and clinical questionnaires sent on an as-needed basis.
A team was created with well-defined roles:
OTH has now grown into a customizable platform utilized by physician groups, hospital systems, and medical device companies. It has been built, managed, and expanded in utility by people who are also its customers and end-users.
OnTrack-Health™’s journey started with Health Care Providers. It was designed by the Providers, for the providers and of the providers. Every aspect of the UX (User Experience) was designed with a strong patient-centric approach, ensuring that unnecessary noise was removed from the platform. The end result is a product that achieved the highest data collection rate in the industry.

OnTrack-Health™ is a platform that provides patients with timely, specific, and peer-reviewed educational resources. These resources help patients understand and familiarize themselves with the procedure, treatment pathway, and devices that will be used during surgery. All of our materials are peer-reviewed and provided by medical professionals and device manufacturers. There are many internet resources available that are not peer-reviewed, causing confusion. Our educational materials are written by professional technical writers, and they are tailored to specific procedures to provide patients with the information they need. Additionally, there is a patient follow-up portal built into the platform that allows physicians and staff, such as nurses and practice administrators, to follow up with patients as needed.
OTH's Surgical Concierge (SC) feature is a mobile application that enables families to track a patient's inter-operative progress during surgery. The family receives notifications as the patient goes through each phase of surgery and receives education about the procedure, customized to the procedure or physician preference. SC can be tailored to a specific procedure, surgeon, or hospital/clinic/surgery center. This feature is immensely popular with patients' families, who receive real-time updates via email or SMS messages with animated videos of the procedures and hospital information at each stage of the surgery.
Data Avenue Aggregation
OnTrack Health™ (OTH) has a significant feature of aggregating multiple avenues of data collection, with each data set being additive and minimal to no overlap. The utility of data avenue is based on the goals of end users. This feature of OTH delivers clear and well-structured outcome analytics, which improves clinical quality and safety. This leads to cost efficiencies for all users, making it an ideal platform for achieving desired outcomes.
OnTrack Health™ (OTH) platform utilizes QR codes to track clinical and satisfaction results for each unique device, patient, and surgeon, attaching to devices seamlessly.
The platform blends customized data streams, reviewing and acting upon patient and physician-reported outcomes, as well as intraoperative experiences.
Real-time aggregation, monitoring, and response to outcome data enable proactive lot recall by device manufacturers, preventing potential FDA fines and revenue loss.
