About Us

Why
OnTrack-Health

The OnTrack Health™ (OTH) platform started with oncologic surgeons who wanted to improve the surgical care of their prostate cancer patients. The proposed idea: a timed pre- to post-surgical pathway incorporating education material and clinical questionnaires sent on an as-needed basis.

A team was created with well-defined roles:

  • Physicians to develop correct procedure timelines and results formats, and assess patient response.
  • Technical writers to ensure clear, concise writing usable across the literacy spectrum.
  • Software engineers to create a robust, secure, and dependable infrastructure.

OTH has now grown into a customizable platform utilized by physician groups, hospital systems, and medical device companies. It has been built, managed, and expanded in utility by people who are also its customers and end-users.

Patient Education/Follow up

Learn More

OnTrack-Health™ is a platform that provides patients with timely, specific, and peer-reviewed educational resources. These resources help patients understand and familiarize themselves with the procedure, treatment pathway, and devices that will be used during surgery. All of our materials are peer-reviewed and provided by medical professionals and device manufacturers. There are many internet resources available that are not peer-reviewed, causing confusion. Our educational materials are written by professional technical writers, and they are tailored to specific procedures to provide patients with the information they need. Additionally, there is a patient follow-up portal built into the platform that allows physicians and staff, such as nurses and practice administrators, to follow up with patients as needed.

Surgical Concierge

Learn More

OTH's Surgical Concierge (SC) feature is a mobile application that enables families to track a patient's inter-operative progress during surgery. The family receives notifications as the patient goes through each phase of surgery and receives education about the procedure, customized to the procedure or physician preference. SC can be tailored to a specific procedure, surgeon, or hospital/clinic/surgery center. This feature is immensely popular with patients' families, who receive real-time updates via email or SMS messages with animated videos of the procedures and hospital information at each stage of the surgery.

Pathways

Procedure Pathways

OnTrack Health™ customizes patient communication for physicians and individual physicians across three phases

Pre-operative Covers events leading up to the surgery day

Peri-operative Encompasses events on the surgery day and subsequent hospitalization

Post-operative Involves events post-hospitalization or post-procedure in an office setting,
with timelines specific to each procedure

Database access and analysis occur through a HIPAA-compliant portal, allowing review at individual, group, or database levels
as per physician and healthcare organization discretion. Machine learning enables predictive models, preemptively customizing
individual patient risks and benefits.

Pathways

Step-Care Treatment Pathways

It is observed that some medical conditions have well-developed step-care treatment pathways, such as urinary incontinence.
To guide and assess patient response and satisfaction with each unique step, platform provides well-structured communication
between patient and provider, notifying providers if and when patients require further care.
This ensures appropriate and customized end points for individual patient treatment, leading to consistent achievement of desired results.
Further, analysis of treatment resolutions can strengthen predictions of response and satisfaction, which can help in refining treatment
protocols. This includes addition of new technologies and identifying their appropriate place on the care pathway.

Navigation Programs

Personalized, timely, and appropriate response to patient reported outcomes improves patient adherence to protocols. OTH Navigator
program allows real-time review and response to patient submissions. Patients are flagged for direct communication for potential need,
and automated response is sent for good progress. If patients do not complete outcome reports, they are automatically notified at
appropriate intervals and flagged for direct communication if required.

OnTrack has become an important part of how I care for patients. My patients and their families appreciate the real time updates and information as they navigate complex pre and postoperative care. I think it has also helped me become a better surgeon by showing me longitudinal outcomes for my patients so I can continue to adapt and improve in areas where I need to. Finally, I think it has become an essential tool during counseling as I am now able to show patients aggregate data on my procedure specific outcomes. I strongly recommend the platform for any surgeon looking to improve care delivery to their patients.

Patients' Testimonials

Your method of texting the patient’s info is wonderful. It is incredibly helpful, and my mind was eased just knowing that everything was going smoothly and that my precious husband was doing fine. Thank you so much

When leaving the hospital the education piece of handling the catheter was quick and short. Thank goodness for the printed material under your reasorces

Very good! Helpful to get texts and the videos. Thanks for your help.

The concierge staff person and the on track program were both very helpful. Thanks for thoughtful, helpful assistance with a surgical day.

I love the fact that I knew every step of my husband’s operation! Every family who has a loved one in surgery should be able to see there step by step progress! So very important knowing and not guessing what is going on in the operating room...less stressful for the family. GREAT HOSPITAL

This was a great tool to have it kept me and my kids well informed throughout the procedure.

Great, informative and efficient tool. Thanks for allowing us to use it. Again, great tool.

It was both fascinating and comforting to see my husband’s progress throughout his surgery process. Thanks

With no visitors allowed during any stage of my husband’s surgical stay (because of COVID) I found it extremely comforting to get the detailed updates. Thank you!
